Step 1: Refer your client through the above link.

Step 2: Once a quote has been submitted and confirmed, an appointment will be scheduled to meet with your client.

Step 3: One of our clinicians will travel to your client to conduct the initial assessment.
​
Step 4: A completed Functional Capacity Report will be submitted within two weeks of the initial appointment.
